| Hi im pregnant with my second after having PE in my first pg. I don't see the cons till 6april when i will be 12wkS and although i have wrote down lots of questions im not sure what to expect or what questions i should ask or should expect to be answed. The only question i really want answering is will this pg be ok and i no no one can answer that. Any info would be grately apreciated many thanks Hannah xx | |

| i just realised that i started writing with intention to ask what the odds would be of me getting PE again? unfortunatly i do have a bit of a weight problem and they did suspect gestational diabeties in my last pg as it also runs in the family thanks again Hannah xx | |

| Hi Hannah,
Welcome to the site & congratulations on your new pregnancy.
Depending on when the PE was first discovered in your previous pregnancy would depend on what kind of treatment you should be offered for this pregnancy. If you were diagnosed & delivered before 35 weeks for example, then a daily dose of aspirin is often prescribed - possibly even heparin. As a general rule though, your care should be consultant lead with increased monitoring. So if you delivered at -lets say - 30 weeks in your first pregnancy, your care for this pregnacy should be along the lines of - consultant appointment before 12 weeks, commenced on aspirin / heparin if blood tests indicate sticky blood. 4 weekly appointments with the MW/GP/Consultant until 28 weeks. then drop down to 2 weeks then weekly by 32 weeks. Regular growth scans - a doppler scan to measure the blood flow across the uterine arteries at 18-22 weeks. Get the consultant to agree a plan that says if your present with higher BP, proteinuria or any of the significant symptoms - what he expects the plan to be. Ask for a copy of this to be kept on your hand held notes. Gestational diabetes can play a part in the re-occurance of PE - so it is really important to keep your sugars stable. BMI also has an influence in the risk factors so again it will be a case of being vigilant & trying to keep your body as strong as it can possibly be. Having said all that PE is so indiscriminate, it could have reared its ugly head even if you had no risk factors at all!
You are correct when no one will say if you will sail through this pregnancy or not, but they can be very vigilant & pre-empt a problem before it gets out of hand. (because you will also be!) Congratulations on your Pregnancy. I just wanted to say My daughter had gestational Diabetes in her second pregnancy but it was no problem in fact she ate healthily as she wanted no problems at all and she lost 2 1/2 stone and looked wonderful ( some weight she has gained since ) thought I would put your mind at rest, Samantha also has High BP problems and is still on medication but this does run in the family on her Dads side but again she was looked after realy well.

| Hi Hannah and welcome to the site. I'm also pregnant with baby no.2. I'm 21 weeks and everything is going ok for me so far. Liz has given you lots of info to go on but one other thing you can ask your midwife for is some dipsticks to check your urine every morning, just to put your mind at ease I hope this pregnancy will be long and boring for you. | Thanks everyone. Sorry im late in getting back to you. Could someone tell me the difference between pre eclampsia and pih (i think i got that right). well when i went to see the consultant he just said that he would send me for a gtt at 28wks and give me an extra scan at 36wks to make sure the baby is not breech also to have my bp checked every two weeks by the mw. I explained that i had alot of head aches and my eyes were constantly hurting and he said he was a little concerned by this and id also had one episode of seeing the little white star things so he told me to tell the mw if this continued in the next two weeks and that he would refer me to an eye specialist as it could be warter retention behind my eyes. As it is still continuing i told the mw last time a saw her and she just said she wasn't concerned and that as i was only 14wks it coudn't be pe. I asked her what she thought it could be that was causing this and she just said it could be because i hadn't eaten enough or was doing too much which to be honest is not me at all i seriously love my food and only do what i have to. Any way i don't realy no where to go from here so any tips or advice would be fab thanks Hannah x | |

| Also i was told my blood was nice and thick which i thought was a bad thing or am i getting confussed with sticky blood? i also thought that it was routine to take asprin and i wasn't told to take that either. I no i should have asked more questions but we had only just come out from the first scan 5mins earlier so was a little bit excited. Im so confussed . | |

| Hi Hannah Sorry for not replying earlier - I'm not sure how much I can say except that I've heard other stories where women (inc me!) had PIH on their notes instead of PE as the reason for early delivery. That upset me at the time, along with many other aspects of my care that I didn't understand or felt/knew to have been substandard.
I think it might be helpful for you to take a copy of the guidelines that you can download from this site (NICE/PRECOG) and show them to your midwife at least, preferably also your consultant. My sense is that you are not being promised an adequate level of care, with just one growth check, and one BP check every two weeks until the end of your pregnancy - as per Liz's answers above. It does depend on when your PE developed, I suppose, but you should get the midwife to check your care plan against the guidelines to see if monitoring should be more frequent.
Can't answer the question about sticky blood - it always confuses me, and my PE happened at a time when all of the stuff about blood was still in the research paper stage...

| Hi Hannah, firstly congrats on your pregnancy. My best guess would be that PIH is pregnancy induced hypertension, but I stand to be corrected on that. If it is then the difference is its not taking into account proteinuria, liver enzymes etc. | |

Latest research shows that if a woman presents with PIH - chances are she will go on to develop pre-eclampsia within 2 weeks which is why the PRECOG guidelines were designed in the way they were.
Hannah, I would suggest checking with your midwife about your 'thick' blood. I dont know when you first presented with PE in your last pregnancy, but again the guidelines suggest that if it was prior to 35 weeks then low dose aspirin is indicated. You might ask what the 'coagulation' screen showed. If it hasnt been done, it might be worth askling for a one off appointment to a haematologist for his/her opinion.
